Consent form

Before you take part in the project you will be asked to complete a consent form to ensure you are aware of how the information you provide will be used.

This consent will be taken through an electronic form. This will be sent to you if you decide to participate in the study. A copy of this form is below, so that you can review the form in your own time prior to the interview.

  1. I confirm that I have read and understand the information sheet for the above study. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily.
  2. I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason, but that my data cannot be withdrawn after anonymisation has occurred.
  3. I understand that any personal information including contact information and personal identifiers will only be accessed by members of the research team.
  4. I understand that I can refuse to answer a question if I want and can stop the interview at any time without giving a reason.
  5. I understand that the research interview will be audio recorded and that the recording will be stored on a secure computer at the University of Bristol and typed up.
  6. I understand that my personal information will be removed from the transcript of my interview and that this transcript may be stored for use in future research studies.
  7. I understand that small parts of what I say may be quoted anonymously when results of the study are reported.
  8. I understand that my anonymised transcribed interview may be shared with other researchers in this country or abroad to support other research in the future, but any personal information that could identify me will be removed or changed prior to making my transcript available.
  9. I agree to take part in the above study.
